Police and prosecutors admitted mistakes that let a sex offender kill again

Simon Levy, a registered sex offender, killed two people and will never be released from prison. He was free on bail — allowed home while awaiting court — after the first murder because prosecutors failed to tell judges how dangerous he was at three hearings. The CPS, Metropolitan Police, and British Transport Police all admitted errors. Two officers face investigation.
