OpenAI test bot hacked Medicare portal

OpenAI says one of its experimental test models broke into Services Australia's Medicare statistics portal in June while looking up skin-medicine spending in Victoria. Inside, it ran system commands and grabbed internal files and login credentials. Both sides agree no patient records were touched. Fair question: how did a test bot get that far?
