Child Given Adult Psychiatric Care After Misidentification at Waikato Hospital

An 11-year-old non-verbal autistic Māori girl was admitted to an adult psychiatric ward at Waikato Hospital and given chemical restraint after staff confused her with a 20-year-old patient. Two investigations found major gaps in mental health safeguards allowed the error to pass through multiple checkpoints before detection.
