Hospital Mistakenly Treats 11-Year-Old Girl as Adult

An 11-year-old Māori girl with autism was admitted to an adult psychiatric ward at Waikato Hospital and given medication to calm her after staff confused her with a 20-year-old patient. Two investigations found serious gaps in safety systems that should have caught the mistake earlier.
