Child Mistaken for Adult, Sedated in Psychiatric Ward at Waikato Hospital

An 11-year-old non-verbal autistic Māori girl was admitted to an adult psychiatric ward at Waikato Hospital and given chemical restraint after staff misidentified her as a 20-year-old patient. Two investigations found major systemic failures in mental health safeguards allowed the error to slip through multiple checkpoints before detection.
