Inquiry finds Oranga Tamariki 'did not listen' on Phillips children

A public inquiry led by senior lawyer Simon Moore KC found Oranga Tamariki, the child welfare agency, missed chances to keep the Phillips children safe. Chief executive Amanda Malu told media at Parliament the agency 'did not listen' as it should have and the family 'deserved better'. She told RNZ she will seek answers on the failings.
