Oranga Tamariki 'did not listen' to protect Phillips children, inquiry finds

Simon Moore KC's public inquiry found Oranga Tamariki missed opportunities to keep the Phillips children safe. Chief executive Amanda Malu told a media conference inside Parliament the agency 'did not listen' in the way it should have and the family 'deserved better'. She told RNZ she will seek answers on the failings.
