Oranga Tamariki missed chances to keep Phillips children safe

An independent inquiry by lawyer Simon Moore KC found Oranga Tamariki, the government agency for at-risk children, missed chances to keep the Phillips children safe. Head Amanda Malu told reporters at Parliament the agency 'did not listen' as it should have and the family 'deserved better'. She told RNZ she will look into what went wrong.
