Letby Inquiry Says Hospital Failures Left Babies Unprotected

The Thirlwall Inquiry says the Countess of Chester Hospital failed at every level to protect newborns. It concludes seven murders and six attempted murders between June 2015 and June 2016 could have been prevented. Its 15 September 2026 report makes 17 recommendations.
