Child mistaken for adult, restrained and sedated at Waikato Hospital

An 11-year-old autistic Māori girl was misidentified as an adult at Waikato Hospital, then physically restrained and sedated. Investigators found systemic failures in patient identification and safety protocols allowed the incident to occur. Mental Health Minister Matt Doocey is seeking a meeting to apologise to the family. Compensation is being considered.
