Aged-care computer tool gets it wrong for one in five

A computer tool that decides aged-care funding gets it wrong in one in five challenged cases. Staff raised the alarm within days of its November launch, five states reported recurring errors, and WA's top official warned of "catastrophic outcomes". The instruction manual was still being fixed in August 2026.
