Anthropic investigating elevated errors on three Claude models

Anthropic posted an 'Elevated errors for multiple models' incident on September 22, 2026. At 00:57 UTC it said it was investigating elevated errors on requests to Claude Mythos 5.1, Claude Fable 5.1 and Claude Opus 5. At 01:17 UTC it said the cause was identified and a fix was underway.
